Privacy – Information Sharing Opt-out Request
If you would like to exercise your right to Opt-out and do not want Bank of Blue Valley to share your personal information with our nonaffiliates to market their products and services to you please provide the information requested in the space provided below. You are required to provide us with only one of your account numbers as a means of identifying you. It is not necessary to submit more than one form. If you choose to Opt-out these preferences will be applied to all accounts you have with us and to all individuals who have joint accounts with you.
